Submission Requirements
Traveler must be able to hit the ground running and need minimal training!  

FLOAT (within 50 miles) may be required for all Dialyze Direct needs. 
  • 1 year or more CURRENT Dialysis experience - Required 
  • Copy of Current, Valid Driver License - Required
  • Geriatric patient experience - Required
  • NXStage Dialysis Machine experience - REQUIRED, Please note experience on submission
  • CNN or CDN Certification - PREFERRED 
  • Crown Web EMR experience - Preferred
  • BSN - Preferred
  • AGENCY NURSE LICENSE (NOT Nursys) REQUIRED FOR SUBMISSION TO THIS NEED!  
  • NXStage Dialysis Machine experience - REQUIRED
